The receiver and programming choices you make on this order form do not need to be your final choices. Room selections needing multiple receivers can result in additional receiver and monthly fees.

3. AGREEMENT

HD Free for Life requires 24 month agreement and AutoPay with Paperless billing.

Programming Package credits.

Programming Package credits: requires a 24-month Agreement and qualifying programming. Must maintain qualifying programming to receive all credits. Upfront Credit: receive a credit for each of the first 12 months after service activation. Online Bonus Credit: when you create a DISH online account, agree to AutoPay and Paperless Billing. You must redeem no later than 45 days from DISH service activation. Must maintain continuous enrollment in AutoPay and Paperless Billing. If qualifying service is terminated prior to end of 24 months, a cancellation fee equal to the lesser of $480 or $20.00 per month for each cancelled month of service will apply.
